  • Submit an application for financial assistance through the “Un chapeau pour ta chimio” program via the Fondation Garde tes cheveux website, under the “Get Support” section.
  • Accept the scalp cooling cap manufacturer’s terms of use and agree to follow the usage protocol that will be explained to you.
  • Use the scalp cooling caps consistently for each chemotherapy treatment for which their use is indicated.

    The equipment required to use the scalp cooling caps is delivered directly to the beneficiary’s home.

    If the caps need to be used for more than 5 months or if costs exceed the maximum financial assistance of $4,000, any extension must be reviewed by the program. The beneficiary will be required to provide proof of diagnosis and confirmation that treatment is ongoing. Any extension of financial assistance must receive prior written approval from the “Un chapeau pour ta chimio” program. Any costs incurred beyond the 5-month period or the maximum amount of $4,000 without prior written approval will be entirely the responsibility of the beneficiary.

    The program beneficiary must have access to a credit card and sufficient funds to cover any expenses not included in the program.

  2. How Is Financial Assistance Provided?

    No funds are automatically paid directly to the program beneficiary. Once an application has been approved, eligible expenses are covered according to the process established between the Fondation Garde tes cheveux, the “Un chapeau pour ta chimio” program and the applicable suppliers. The Fondation Garde tes cheveux is responsible for managing and disbursing the funds allocated to the program.

    • understand that scalp cooling caps do not guarantee complete hair preservation;
    • agree to follow the manual scalp cooling cap usage protocol;
    • will independently manage the equipment entrusted to you during your treatments. You are responsible for the equipment and may be charged in the event of loss or major intentional damage;
    • will arrange, whenever possible, for someone to accompany you and assist with the cap changes required to properly follow the protocol;
    • will promptly notify the program if you stop using the caps or if your situation or treatment protocol changes;
    • agree to return the equipment to the manufacturer no later than 5 business days after the end of your treatments. Otherwise, additional costs may be charged to you;
    • agree to comply with the manufacturer’s terms of use, including the prohibition against subletting or lending the equipment.

  5. Allocation and Availability of Financial Assistance

    Eligibility for the program does not constitute an automatic guarantee of funding. Financial assistance is awarded in accordance with the program criteria and based on the funds available at the time of application. Any financial assistance provided must be used exclusively for eligible expenses related to the use of scalp cooling caps under the program.

    The “Un chapeau pour ta chimio” program is responsible for reviewing applications and approving them jointly with the Fondation Garde tes cheveux. The Fondation Garde tes cheveux retains responsibility for the management, approval and oversight of the funds raised and their use.
